2008 LMYFA Registration

We are still accepting registrations for cheer and football.

2008 Season Registration is ongoing and we are accepting regisrations for all levels of cheer and football. The registration fee is $285.

A previous version of the registration form included an incorrect PO Box number. The correct address for LMYFA is PO Box 951901, Lake Mary, FL 32795.
The registration form linked below is correct.
If you have submitted a registration form more than 1 week ago and have not heard from your coach, please contact us immediately at commissioner@lmrams.com.

To register follow the steps below:

 

1) Consult the boundaries map to ensure you are within the LMYFA zone.

2) If you are within the LMYFA boundaries, print the Registration form (a.k.a. Player Contract/Consent Form.) 

 

3) Send the completed the Registration form and registration fee (check or money order - do not send cash) to:

REGISTRATIONS MUST BE MAILED SINCE WE USE THE POSTMARK TO DETERMINE THE RECEIPT DATE.  ATTEMPTING TO HAND DELIVER THE FORMS MAY ACTUALLY INDUCE DELAYS WITH RESPECT TO THE RECEIPT DATE.

 

3) Please note the following:

Make sure that you provide all the requested information on the forms and don't forget to check the program you want to participate in (Football, Cheer or Dance)! Required fees are noted on the Registration form.

• DO NOT send physicals, report cards or birth certificates in the mail. You will be asked for these items once the participant is placed onto a team.  Also, keep a copy of these items for your records since LMYFA will not be able to return copies back.

• Teams are formed on a first-come, first-serve basis, based on registration postmarks. Registrations for new participants received prior to April 1, 2008 will be considered as postmarked on April 1st. Participants will be notified as to the team/squad they are placed on, or if they are placed on a waiting list.
  

  To complete the registration process, some other items will be required before practice begins:

A physical obtained during calendar year 2008 using the official, 2 page physical form found below. No 2007 physicals will be accepted. No player will be allowed to begin practicing without a completed physical form.

A copy of the participant's birth certificate, and


Three copies of their final report card (complete 2007/2008 school year).

All required paperwork and the balance of the registration fees must be provided prior to equipment/uniform fitting.  The coach/team parent will let you know when to provide these.

REMEMBER: INITIALLY ONLY SEND IN THE COMPLETED REGISTRATION FORM AND DEPOSIT.  YOU WILL BE ASKED FOR THE PHYSICAL, REPORT CARDS AND BIRTH CERTIFICATE AT A LATER DATE!

 

Special Note Regarding the Medical Form: Please have a physician complete & sign the page 2 of the form. The first page of the form is to be completed by the Parent/Guardian. It is very important that ALL questions be answered in the top section (if a question does not apply then write "N/A" in the box).
