2012 LMYFA Registration

Registration for the 2012 Football and Cheer season is open for all returning cheer and football. New participant registration will be on February 22nd. Teams will fill up fast so don't delay. Roster spots will be filled by the dates complete registration forms and full payments are received. All others will go on the waiting list.

To register follow the steps below:

1. Consult the boundaries map to ensure you are within the LMYFA zone.

2. If you are within the LMYFA boundaries, print the Registration form (a.k.a. Player Contract/Consent Form.)
3. Send the completed the Registration form and registration fee (check or money order - do not send cash) to:

   Registrations must be mailed or brought to a dedicated sign up day. The postmark or sign up days are used when teams are full.

Please note the following:

• The first $50 of all registrations are non-refundable
• There are no refunds after May 1st
• Make sure that you provide all the requested information on the forms and don't forget to check the program you want to participate in (Football or Cheer)!
• DO NOT SEND PHYSICALS, REPORT CARDS OR BIRTH CERTIFICATES IN THE MAIL. You will be asked for these items once the participant is placed onto a team. Also, keep a copy of these items for your records since LMYFA will not be able to return copies back.
• Teams are formed on a first-come, first-serve basis, based on registration postmarks. for new participants received prior to March 1, 2012 will be considered as postmarked on March 1st. Participants will be notified as to the team/squad they are placed on, or if they are placed on a waiting list.

To complete the registration process, some other items will be required before practice begins:

• A physical obtained during calendar year 2011 using the official, 2 page physical form found below. No 2010 physicals will be accepted. No player will be allowed to begin practicing without a completed physical form. (We understand that some insurance companies will not pay for more than one physical per year. Opportunities for reduced rate physicals will be announced for families in this situation.)
• A copy of the participant's birth certificate
• Three copies of their final report card (complete 2010/2011 school year)

All required paperwork and the balance of the registration fees must be provided prior to equipment/uniform fitting. The coach/team parent will let you know when to provide these. Refer to the information found on the Age/Weight Requirements page (link on the left) for more information regarding the teams the Lake Mary Rams will field.

Registration Forms- Note that you must have Adobe Acrobat installed on your system in order to download the forms. If you do not have Adobe Acrobat, go to www.adobe.com and click on the "Get Adobe Reader" icon and follow the directions. The Adobe Acrobat reader is free!

Special Note Regarding the Medical Form: Please have a physician complete & sign the page 2 of the form. The first page of the form is to be completed by the Parent/Guardian. It is very important that ALL questions be answered in the top section (if a question does not apply then write "N/A" in the box).
